THE SWING BED PROGRAM

Contact: (814) 723-3300 ext. 1789

Typically after an illness, injury or surgery you may not have the ability to return to everyday living. The Swing Bed Program at Warren General Hospital gives you the opportunity to recuperate in a hospital setting under the supervision of our skilled physicians, nurses, and staff.

The focus of the Swing Bed Program is you -- the patient -- and making sure that you receive the proper rehabilitation to ensure a safe and sustainable return to home.

How does it work?

After you have been an inpatient at Warren General Hospital, a group of physicians, nurses, social workers and other staff work with you to determine an individualized plan of treatment that is best suited for your recovery. Over the course of your treatment plan, our exceptional staff will care for you around the clock until you are discharged from the hospital.

Swing Bed Services

Skilled Nursing

A skilled nurse will assess your medical care needs. They will develop and manage a plan of care in coordination with our multidisciplinary team.

Physical Therapy

This type of therapy provides you with the ability to increase strength and endurance while improving mobility, balance, posture, and coordination following an illness, injury, or surgery.

Occupational Therapy

This therapy will help you build or regain your abilities to perform the tasks associated with independent living with emphasis on your safety and fall prevention.

Speech Therapy

Speech therapy provides speech and language treatments to both children and adults. Pathologists evaluate and treat speech disorders caused by stroke or illness as well as treatment for a variety of cognitive and/or swallowing concerns.

Social Services

Social workers assist patients and families in helping them cope with the social, emotional and economic challenges that a medical condition can pose, establish a long term plan and ensure that healing is taking place in a safe and appropriate environment.

Discharge Planning

We will work directly with you to determine this course of care, which may include Home Health Care, Outpatient Rehabilitation Services, or Nursing Home Placement.

Swing Bed Program Eligibility

This program is routinely considered for those who need short-term extended care that will assist in recuperation. Eligibility for the Swing Bed Program will be dependent upon your physician along with other members of your healthcare team.
