Hospital Home Care offers skilled nurses, physical therapists, occupational therapists, speech therapists, medical social workers, and physical therapy assistants that provide quality home care.
The HHC team helps coordinate medical treatment, including patient assessments; developing care plans, monitoring medication compliance, and more. Relationship building with patients is their strong suit, serving as advocates for and champions of the patient’s health.
Through collaboration with patients' families, friends, social support, and physicians they help develop treatment plans. This collaboration with healthcare providers helps facilitate and ensure patients' treatment plans are met.
